I will be posting a freebie in here in just a minute, but in case you are new to this whole requesting thing, I decided to put up an example form of how it will need to look when you request something from me. That way I have all your information to create your graphic :)

Name on graphic: {your desired personal name will go here}
How many boxes: {if it's a blinkie, I will need to know the # of letters in your name for the boxes}
Hair color: {your personal hair color will go here}

That's pretty much it..and easy peasy to follow :)
